- NEET UG 2026 Scorecard / Admit Card
- Class 10 Certificate (DOB proof)
- Class 12 Marksheet + Certificate
- Transfer Certificate (TC) from last school
- Category Certificate (OBC / SC / ST / EWS) — if applicable
- PwD Certificate — if applicable
- ID proof (Aadhaar / Passport / Voter ID)
- Passport-size photographs (10+)
- MCC Registration & Allotment letter (printout)
- Anti-ragging undertaking (signed)
- Medical fitness certificate from a registered MBBS doctor
Carry originals + 3 self-attested photocopies of each document to the reporting centre.
